Tender Care in the Comfort of Home

When you are recovering from a surgery or managing an illness, you need that gentle touch… that tender care. You want the gentle reassurance that you can live safely and independently at home.

Sometimes you need the care of skilled medical professionals to help with your healing, especially when leaving home is difficult. TenderCare Home Health can help you heal in the comfort of your own home.

Maybe it’s a short-term need like recovering from joint replacement surgery. Or perhaps you have an ongoing need for help with heart problems, diabetes, breathing difficulties or other issues.

TenderCare Home Health provides care from skilled nurses, therapists and other professionals. The caring and highly-trained clinical team will:

  • Help you manage health concerns, like heart failure or joint replacement,
  • Work with your doctor to develop a custom plan for your care,
  • Assist you with medications and nutrition,
  • Coordinate medical equipment, like wheelchairs or diabetes supplies,
  • Identify and advise you on home safety hazards,
  • Help you or your loved ones live safely and independently at home.

Medicare Coverage

Everyone with Part A and / or Part B Medicare is eligible for home care coverage if you meet these requirements:

  1. You must be homebound, or leaving home is a major effort. For example, you have trouble leaving your home without help (like using a cane, wheelchair, walker or crutches; need special transportation or help from another person) because of an illness or injury. Or maybe leaving home isn’t recommended because of your health condition.
    You can still qualify for home health if you leave home only for short periods -- like going to your doctor or attending religious services – or if you attend adult day care.
  2. You must be under a doctor’s care, and have a plan of care established and reviewed regularly by a doctor. Our agency will work closely with your doctor on a plan customized for you.
  3. You must need at least one of the following: part-time skilled nursing care, physical therapy, continued occupational therapy or speech / language therapy. Part-time or intermittent nursing care means less than 7 days each week or less than 8 hours each day.
  4. The home care agency must be approved by the Medicare program. TenderCare Home Health is Medicare-certified.
  5. You must have a face-to-face visit with your doctor 90 days before or 30 days after beginning home care services.
